We have been closely working with our partners for several months to enable a smooth transition of services for patients and staff that ensures the delivery of high quality community services that supports the health and wellbeing of local people.
“We’re committed to achieving the vision set in the Shaping Sefton programme and want to see a better join up of all healthcare with other organisations that are often involved in providing an individual’s care. The aim is to provide services in a more seamless way and close to people’s home whenever it is appropriate and safe to do so.”
